As your case management agency, we will assist you and your loved ones with the following:

1. Referral for the Elderly and Disabled Waiver Program 
2. Completing an assessment performed by a nurse to determine functional and financial eligibility for the program
3. Submission of functional assessment with supporting medical documentation to the state of Georgia for approval 
4. Applying on your behalf to the Georgia Division of Family and Children Services for CCSP (Community Care Services Program) Medicaid which will aid in paying for the program’s services
5. Monthly contact and monitoring of ordered services and clients’ level of care to maintain that all services are being rendered as ordered and that the client continues to benefit from community services
